Contact us immediately on 0800 374 626. You will be asked to send us the documents we need to respond on your behalf, including:
- a copy of the letter you have received from the patient's solicitor
- a signed note asking us to assist you
- the original records or clear and complete photocopies
- a factual report of what happened and the names of other dental professionals involved.
Time is of the essence. Records must be disclosed within one month of the solicitor's request.
